You have to go to the website for their broadcasting network for your area.. it will direct you to a link to print off your application to be a participant..
I wish I knew the link to give to you.. I have personally handed out forms to others for this reason.. since I knew a lady that lost her husband suddenly and didn't want to move.. but she didn't have enough space for her family to all visit during holidays..
